Old School RuneScape Bug Let Bosses Double-Kill Players
Old School RuneScape just learned the hard way that a friendly tutor can still cause a lot of grief.
A recent patch added an NPC whose whole job was to teach players the basics of PvP in the sandbox. On paper, it was a helpful idea. In practice, the update that brought him in also shipped a bug that let bosses kill players twice in a row. The second death then destroyed whatever gear those players had been waiting to recover.
How dying works in OSRS
To understand why this hurt so much, you need to know how death works in Jagex's MMO. When a character falls anywhere outside a safe zone, they have 15 minutes to get back to their grave marker and pick up the items they dropped.
Certain boss fights handle this differently. They use their own death storage, called item retrieval. Instead of a grave, the items lost in the fight wait in a chest or with an NPC outside the encounter, ready to be claimed. There's a catch, though. If the player suffers another unsafe death before collecting that stash, the stored items are gone for good.
Most of the time, that rule works as a fair warning. This week, it became a trap.
Dead twice, robbed once
Players soon started flagging that boss deaths were behaving strangely. Normally, a fallen character gets kicked out of the arena. Instead, characters were staying inside the fight after dying, getting killed again almost immediately, and only then being moved out. The game treated that as a second death, which wiped the first item retrieval inventory. With it went piles of extremely valuable equipment.
The Reddit thread that reported the problem summed it up in all caps: "DIED WITHOUT DYING AND THEN DOUBLE DEATH." It's hard to describe it more accurately than that.
Jagex moves quickly
To its credit, Jagex didn't let this linger. The studio confirmed the bug, took the affected worlds offline to deploy a fix, and then brought them back up. It also began working to restore the items players had lost to the unfair deaths.
Crucially, it did all of this without a rollback. That matters. A rollback would have erased legitimate progress for everyone else on the servers just to undo the damage for the people who were hit. Restoring items directly is the slower, more careful route, and it spares the wider player base a lot of frustration.
A bigger argument about death penalties
Fixing the bug didn't end the conversation, though. It started a bigger one about the death penalty itself. Several players argued that permanently destroying gear after a second death is simply too punishing, glitch or no glitch.
Jagex appears to agree. "We fully intend to get to this and are exploring it as the next direct focus for QoL team," one of the developers wrote. "It'd have been nice to have had this done before today... perfect highlight of why we should get this tidied up."
It's unusual to see a developer admit so openly that a bug exposed a design weakness rather than just a broken line of code. But that's exactly what happened here. The glitch didn't invent a new way to lose items. It just triggered an existing rule at the worst possible moment, and at scale.

For anyone following Old School RuneScape's recent updates, this episode looks like a familiar live-service problem. A small, well-meaning addition can have unexpected effects on systems that seem completely unrelated. The quick fix and the decision not to roll back suggest that Jagex knows how much trust is at stake when valuable gear disappears through no fault of the player. That kind of goodwill matters for Jagex right now.
The more interesting question is what comes next. The developer's comments point to item retrieval being the quality-of-life team's next focus, but no changes have been announced yet. It is worth watching whether Jagex softens the second-death rule or simply adds safeguards around it. Old School RuneScape players tend to value risk, so any change will need a careful balance.
